Rehearse:-

If you have fear, then start rehearsing your presentation or speech and have command on your words and the way you speak them. Stand in front of a mirror or get your family to sit in front of you and then deliver your speech. That’s how you gain confidence and control over your fear.

Enough knowledge:-

The speaker should have enough attention-seeking information regarding his or her topic. You should go through and find the best recent updated researches or material about your topic. This will not only give you the attention, but also you will be comfortable and confident while speaking when you will have all the eyes on you. Don’t be overconfident. Speak what you know; do not add additional or annoying things. Present the facts that the audience doesn’t know.

Use imagination:-

Psychology says that a person can say or do things more perfectly the way he or she has imagined. Visualization and imagination can turn things differently. Imagine the things and scenarios you are about to tell or present. This will help you in giving a friendly, confidential speech.

Admit nervousness:-

If you are getting nervous, rather than fainting or anything, take a deep breath and confess in front of the audience that you are getting nervous. That will help in many ways, like you will gain empathy or comments so you can be friendly and relax. And after such empathy, it will help you to calm and revive from any anxiety.

The Tone should be friendly and formal:-

The speaker’s tone of speaking should be friendly. He must welcome all of his audience. And speak in a good engaging tone so the audience should enjoy and take part in listening carefully to what he speaks. The speakers with boring tone or rude tone do not get much affection and respect.

Making mistakes and joking in between:-

For having control of the audience, you should actively engage with them. Make mistakes intentionally like dropping off your notes, sudden switching off the presentation, or any other joke wherever you think that the speech is going bore. This will help in refreshing the audience and give you time to gain their attention and control. Casually, doing such things make you feel easy and comfortable to deliver. And also make it fun, memorable, and enjoying the people listening to you.

Prefer short speech than long:-

Long speeches or presentations bore the audience. They find it irrelevant. The shorter the speech is, the more authentic it will be. And it is also followed by more questions that will awake the audience to ask more. Talk less but effective. It improves the quality of the speech and authenticity of the topic. The speaker should be to the point with the audience and do not lie about anything.

Give references:-

An Authentic speech should contain references from where the matter is taken and extracted. This works as a confidential proof that the information given is authentic, and there is no lie in it. And it also gives quality to your speech. Because more than quantity, quality matters. The more effective references would be, the more quality will your speech has.

Share your experience and personal opinion with the audience:-

Try sharing your own experience with the audience. Sometimes the audience gets attracted to the personal opinion and experience of the speaker because it helps them to gain more knowledge. If it is interesting, then they will have the urge to experiment with it too. This will also add fun to the speech. And also good quality time with your audience.

Before you leave:

It is normal and natural human nature to feel frightened for the first time when you speak in a large audience or high-level judges. With all these tricks, there are some essential things you should have in mind if you want to be a good speaker. Welcome everybody with a smile. As we all know that “SMILE IS THE UNIVERSAL WELCOME.” We should be friendly with the audience and talk person by person and engage all of them in our talk. Another thing is that don’t apologize for the mistakes while speaking because usually, people don’t notice that, so instead of offering apologies, continue speaking and make it a fact of fun.

I hope you enjoyed this and these tricks help in getting your speaking skills better and overcoming your public anxiety and social fear.

It reacts to the accelerations of the vehicle, the fluid inside the hollow rim moves around the eyes in the frontal direction (left and right), and in the sagittal direction (front and back).

It creates an artificial horizon in the Visual field, without disturbance in the central vision. The eyes then have access to the reality of the moment, the sensory mixing disappears, and the travel sickness resolves in a couple of minutes.

While the sources are still not entirely understood. According to the Citroen company, these glasses work through the technology use of the Boarding Ring developed with the same name that is licensed to Citroën. These glasses work exactly like Boarding glasses by creating a “virtual horizon” which is simulated with the help of the liquid in each of the four rings, helping the wearer’s eyes match what their vestibular system is detecting and signaling to the brain. This can solve the sensory mixing problems.

The Citroen company claims that after 10 to 12 minutes of wearing the Seetroen glasses and enable the mind to synchronize with the signal perceived by the ear while the eyes were on an immobile object such as a phone, laptop, or a book. Take the glasses off when the symptoms stop and enjoy the rest of the journey.

Are you familiar with Aphasia and Dysphasia?

Dysphasia

You have seen many children and adults being bullied on impaired or stammering ways of speaking. You were not able to speak properly. Have you ever noticed this type of thing in your children, or are you confused with these terms? Scroll down and see what this means and how can you notice that any child or any adult has aphasia or dysphasia.

Let’s talk about what is aphasia and dysphasia—removing the confusion within these terms. These are both technically psychiatric disorders. Damages or injuries to the patients cause that. Sometimes it is caused by different problems. It acquires in children at an early age, but if not treated at the start, then it can continue as the child will grow.

Definition Of Aphasia:

Aphasia is known as the person’s inability to comprehend or speak something. The inability to write or read something. This is technically a communication disorder. These problems appear as a result of brain parts damage, those parts which are responsible for speech and language. This condition happens suddenly after brain damage or sudden brain injury, a stroke, or head injury.

Aphasia interrupts a person’s ability to understand language, to speak properly, or to write. Aphasia does not impair intelligence but impairs the thoughts of a person as the person is unable to use the right words for what he is thinking or speaking. Aphasia can be mild and can be serious that the person is unable to speak.

Causes of aphasia:

The injury of most parts of the left hemisphere is usually the cause of aphasia. Any damage or disorder to the parts of the brain controlling speech, communication causes aphasia. Damage of deep subcortical tissues of the brain also gets in line for aphasia. Some of the common causes of aphasia are:

  • Stroke
  • Paralysis
  • Brain tumour
  • Traumatic brain injury
  • Alzheimer’s disease
  • Neurological disorders
  • Atrophy in brain tissues
  • Dementia
  • Epilepsy
  • Infectious diseases
  • Gunshot wound
  • A Side effect of an opioid drug, fentanyl.

Signs and symptoms:

People with aphasia develop behavioural changes because of brain damage. The symptoms of aphasia depend upon which part of the brain is damaged. In some cases, signs and symptoms do not appear in the beginning or in the mild stage but can develop with the age or as the wound gets deep. Sometimes people hide this due to shame and stop communication. This is the worst part from here double damage starts and it’s hard to be treated. Symptoms of aphasia which are noticed commonly are:

  • Unable to make words
  • Unable to read words
  • Unable to write anything
  • Unable to speak words or sentences
  • Unable to pronounce names.
  • Anomia (unable to Name objects)
  • Unable to repeat something which is said before.
  • Repetition of a single word or phrase
  • Speech automatism
  • Paraphasia
  • Agrammatism
  • Limited speech
  • No fluent speech or flow of words
  • Speech disorder
  • Struggling in finding correct words for telling the thoughts or explaining something.
  • Pronouncing strange words

What are the types of aphasia?

The types of aphasia are due to its severity from mild to extreme impairment of brain function. Mainly it is divided into two types 1. Fluent, and 2. Non-fluent.

These types and patterns of aphasia vary from person to person and categorized from the location of brain injury in each case.

  1. Type #1 is Global Aphasia
  2. Type #2 is Broca’s Aphasia, also known as “non-fluent aphasia.”
  3. Type #3  is Mixed non-fluent Aphasia
  4. Type #4  is Wernicke’s Aphasia, also known as “fluent aphasia.”
  5. Type #5  is Anomic aphasia
  6. Type #6  is  Primary progressive Aphasia

Aphasia can be mild or severe. This depends on the type of brain injury and which part is how much damage. In cases, if the patients stop communicating or hide this problem, he or she develops complex aphasia syndrome in isolation. This affects the ability to read, write, speak. And they are then followed by a stroke or dementia. The person should try to talk and converse with the people if still, the person has trouble speaking or understanding but should try and should participate.

Severe impairments happen but if it is diagnosed at the right time and retained speech, excellent results are shown.

How can it be treated?

Therapies come first in line for speech and language disorders. But the treatment is suggested upon these factors.

  • Cause and reason for the brain injury
  • Size of the damaged area
  • Age of the patient
  • Type of Aphasia.

In some cases like cancer or tumour, if these are developing in the part of the brain where speech is manufactured, then surgeries are the only option to retain communication.

For the people with a neurologic disorder, stroke, epilepsy, speech therapies, and language therapies create a good impact on the affected brain part. Some are learning the language in which speech is not involved also helps the person to explain his thoughts and act according to it.

Drawing pictures, use of hands also helps the patients to communicate and make him understand what is happening with him. This relaxes the patient. Make him comfortable that he or she speaks slowly and does not get excited while speaking. The more patients stay calm; the more effective treatment will be.

Make cards, use props for the affected person to show his feelings.

The last option is medications, drugs.

Intensivist: Why Intensivist Differ From General Physicians?

Intensivist: Why Intensivist Differ From General Physicians? 18 - Daily Medicos

With this word, many questions arise in mind. Many people don’t know what an intensivist is or have doubts regarding their job and duties. By the end of this article, we will get to know about intensivist or intensive care practitioners.

What is an intensivist?

An intensivist is a physician, a board-certified physician who is specialized in most ICU practice and providing specialized care to critical patients and emergency patients.

They are specially trained in skills to treat complex patients.

Critical care physicians are also called intensivists as they give care towards complex cases.

They are professionalized in intensive care or critical care medicine.

These type of physicians have very high duty on their shoulders, as they have to manage patients with low mortality rate and unwell condition. They have to be very careful in handling patients, as many of them have acute illnesses with chronic. Such patients have low stability rates as well and involve in extreme clinical problems that may lead to death.

An intensivist is a need of every hospital containing trauma center or ICU because an intensivist is specialized in these kinds of skills and can give quality care to patients, reduce their time for stay, improve their survival chances as they know multiple parameters regarding respiratory distress,  and reduce complications which can be challenging for a general physician.

Usually, there is a shortage of intensivists in most of the hospitals around the world, and the need for intensivists is emerging day by day.

What kind of training intensivist have?

An intensivist is trained in special skills for handling critical patients or patients in the critical care unit.

An intensivist is trained in either adult or pediatric intensive training, which has different parameters.

They have a specialty in cardiac, pulmonary surgeries, and medicine, which include most of the respiratory issues that an intensivist came across most of the time.

After doing fellowship and residency, they get further certified in critical care medicine or intensive care medicine in either adults or pediatrics.

Institutions rewarding :-

In Pakistan Multiple renowned institutions like

Aga Khan Medical College is also providing a program in ‘CRITICAL CARE MEDICINE’ after residency.

Dow University of Health Sciences is also providing degrees and certifications in ‘CRITICAL CARE’ specialty.

In the United States,

Stanford University is providing fellowship in ‘ACUTE CARE MEDICINE’ and ‘CRITICAL CARE MEDICINE’.

The University of Toronto has a fellowship program in critical care medicine as ‘INTERDEPARTMENTAL DIVISION OF CRITICAL CARE MEDICINE (IDCCM).

In Pennsylvania, a reputed institute, Geisinger medical center is giving fellowship experience to residents in ‘CRITICAL CARE MEDICINE’.

The University of Pittsburgh is also providing a fellowship program in ‘INTERNAL MEDICINE’ and ‘EMERGENCY MEDICINE IN CRITICAL CARE’

American College of Physicians is also providing programs in ‘CRITICAL CARE MEDICINE’.

Not just Pakistan and the United States but worldwide countries are providing fellowships and experiences in critical care, acute care, intensive care in adults and pediatric both with setting up models and projects and physicians and health care providers like nurse, technicians or technologists are involving and taking part with great interest.

Why Intensivist differ from general physicians?

General physicians do work on all the body systems usually they are primary care doctors as for an intensivist they are specialized in certain skills that handle certain complex patients and provide care to critical patients in a certain environment.

Intensivists don’t go for specific body parts but usually trained for critical patients and unwell conditions.

They have to make last moment decisions and often leads a team for doing measures for that critical patient and oversees all the condition going on with that critically ill patient and what other medication and services he/she needs.

What time required to become an intensivist?

For a certified fellowship of becoming an intensivist is of about 3 to 4 years and for doing certification in critical care medicine altogether, it takes about ten years with a further degree and certified training in skills and medicine.

Job description for an intensivist:-

An intensivist gives primary care for the post-operative, critically ill, and traumatic patients who need life support and specialized care.

They give care to patients with distress and having cardiac and respiratory issues.

The patients having breathing problems also come under an intensivist as it gets complicated and needed life support on getting worse.

An intensivist can give quality treatment to ICU patients because they have specific knowledge for handling such patients.

They provide care to :

  • Intensive care patients.
  • Pediatric intensive units.
  • Medical intensive care units.
  • Surgical intensive care units.
  • Neonatal intensive care units.

As per their specialities.

What type of procedure an intensivist can perform?

An intensive can lead or perform any procedure needed for the critical patient like :

  • Dialysis
  • Intubation
  • Ventilation
  • Tracheostomy
  • Central venous access
  • Arterial cannulation
  • Drainage procedures
  • Echocardiography in ICU
  • Echocardiography in ICU

According to another web page, ‘HEALTH CARE WORK SALARY,’ the yearly salary for an intensivist in the United States of America is an average between $202k to $392k.

What is an intensivist nurse practitioner?

Nurses are very crucial to the health care department. They are the backbone of hospitals providing sudden and emergency care to patients. Every patient has a nurse or team of staff who take care of them provide proper treatment and medication as per the directions given by a physician—minute to minute monitoring and management of the patient.

An intensive care nurse seeks an important role in an ICU model as they have to monitor critical patients. They are highly skilled in giving care to patients with life-threatening diseases.

They worked with tertiary care teams and prepared for all practices.

An intensive nurse focuses on giving care to patients of cardiac arrest, shock, trauma, sepsis, brain injury, stroke, respiratory distress, organ failure, etc.

Salary of an Intensivist nurse practitioner:-

The average salary of a nurse practicing in ICU or critical care setup makes about $86k to $125k given by the website ‘PAY SCALE’ excluding profit and bonus.

Pediatric Intensivist :-

A pediatric intensivist gives care towards critically ill infants and children in the health care system and monitoring their life support and basis of life. They supervise the care of children in emergency conditions by providing proper treatment saving their lives and providing care in heart, lung injuries, or abnormalities.

A pediatric intensivist is a need for all the hospitals worldwide as they know the proper handling for child and infant and have ultimate knowledge for seriously ill infants and children who have less stability and low mortality rate. They care for many things regarding the child patient. They supervise all the procedures regarding that infant and manages all the services.

They have the authority to perform sudden surgeries or procedures required or needed by the patient.

Salary for an pediatric intensivist:-

Average of about $305k is made by a pediatric intensivist in the US  per year, given by ‘COMPARABLY’.

Thyroid Cartilage

Thyroid Cartilage 20 - Daily Medicos

The front set of plates in the larynx is known as thyroid cartilage; these two plates are known as ‘laminae.’ The name of the thyroid cartilage is because of its sharper edge rather than on the thyroid gland. In the Greek language, thyreos means ‘SHIELD SHAPE.’ The thyroid cartilage has two plates fused, an elevation, and a central ridge known as Adam’s apple. The fusion can be easily felt through the skin. It is also known as the largest cartilage of the larynx. Behind the shield, there are vocal cords. Posteriorly the borders of the thyroid cartilages look like horns, which are upward and downward.

The superior upward horns attach by the thyrohyoid bone to the hyoid bone and inferiorly the horns are attached to the cricoid cartilage at the cricothyroid joint.

The angle between the two plates of the thyroid cartilage is sharper, and it has seen more prominent and visible in men instead of women. The lower-pitched voice in males is also due to vocal cords in the larynx. The thyroid cartilage grows with age, especially visible in puberty. The thyroid cartilage does nothing with the gland but plays a role in protecting the vocal cord and maintaining pitch of voice and its production. Thyroid cartilage has nothing to do with the thyroid glands.

Thyroid gland:

The thyroid gland is an endocrine gland that is located in the neck.

This gland produces two hormones:

  • T3 (triiodothyronine)
  • T4 (thyroxine).

These hormones contain atoms of iodine. T3 contains three atoms and T4 contains four atoms of iodine. These biologically active atoms help the tissue work properly.

This gland is butterfly-shaped. It lies below the adam’s apple (the laryngeal prominence produced by the angle of thyroid cartilage). Hormones released by the thyroid gland are essential for the cells of the body to work. There are two lobes of the thyroid gland which lies above the windpipe.

Thyroid disorders mainly occur in women. But men, children, and adults can also develop this condition.

What thyroid hormones do?

Thyroid hormones maintain the metabolism of the body, which regulates the speed of working of the body. If the release of these hormones gets fast, the body starts working faster, which converts in an abnormal condition known as ‘HYPERTHYROIDISM.’ And if the hormonal release gets slow, the body slows down the work and develops a condition called ‘HYPOTHYROIDISM.’ These conditions lead to abnormal i.e., the fast heartbeat in hyperthyroidism and slow heart rate in hypothyroidism followed by increased activity of different organs as well as bowel movements and constipation in hypothyroidism. The most commonly developing condition is hypothyroidism.

Symptoms of thyroid disorders:-

For hyperthyroidism: weight loss, anxiety, sore eyes, fast heart rate, diarrhea,  increased bowel movements.

For hypothyroidism: weight gain, depression, tiredness, slow heart rate, constipation.

Thyroid diseases can be treated and also can be prevented by taking supplements or medications prescribed by the physician.

Emetic: Homemade Emetics and Ipecac

Emetic: Homemade Emetics and Ipecac 25 - Daily Medicos

Homemade Emetics and IPECAC Details Explained!

As we know that there are many drugs made and discovered for multiple causes and diseases. Drugs are used to minimize the effects of abnormal things or disease conditions developing in the body. Just like these, scientists have made emetics that are medications that can be used to cause vomiting or make a person vomit if she or he has engulfed something. For example, if somebody took poison or took any wrong medicine so emetics are given to take them out before the adverse reactions appear or the condition gets worse. In medical terminology ‘emesis’ meaning ‘vomiting’. And the drug which induces vomiting is  ‘emetic’.

What are Emetics?

An emetic is a medication or agent which is used to cause vomiting or nausea. Emetics are widely used throughout the world. People who have children especially have emetics in their first aid kit for an emergency if the child engulfs any toxic item or any poison or anything from which he is allergic. It is like a potion or syrup which directly acts on the gastrointestinal system.

What emetics do?

Emetics forces the stomach to empty itself. It is used in emergency cases of poisoning or intaking any toxic or harmful substance. The main function of emetic is the sensation of nausea and the ability to vomit the toxins which are engulfed. Emetic should be taken after reporting intake of poison before it gets fully ingested and give dangerous and harmful symptoms. The use and function of emetics are limited to induce vomiting and take the poisonous things out. If they are used for any other thing then it could give bad reactions.

Emetics causes dizziness, nausea, vomiting obviously, fast and slow uneven heartbeats, irritation in the stomach, shortness of breath in cases, and low blood pressure.

Homemade emetics:-

Homemade emetics are usually prepared by the dried roots of plant ‘CARAPICHEA IPECACUANHA’ which is commonly found in Brazil, Central America, and Atlantic forest. The most common emetic syrup is ‘IPECAC’ and it can be prepared homeopathically by the root of ipecacuanha. The syrup stimulates the nervous system and forces the stomach to take out everything.

The main purpose of this syrup is to expectorate ingested poison or drug overdose from a person or any other noxious thing before fully ingested. It is a powerful emetic that is also used for the treatment of gastrointestinal diseases, and diarrhea in some cases. Knowing how to make an Ipecac preparation will help in cases of emergency situations such as drug intoxication or if somebody engulfed poison.

Parents for years, usually keep this emetic syrup at home to induce emesis.

How to prepare homemade ipecac:-

These instructions can be used under the assistance of any professional. Kindly keep it out of the reach of children. Your first priority is to buy this emetic from a medical store if you can not find it there then you can make a homemade emetic with the help of the following items and step by step procedure.

What ingredients do we need?

For making homemade ipecac syrup for emesis, you need:

  1. Water
  2. Acetic acid
  3. Glycerin
  4. And sugar.
Procedure:-

The step by step procedure for making homemade emetic that is ipecac at home is as follows.

  • First, add 1000 ml of water with 10 ml of acetic acid and put it aside.
  • Then Dilute the ipecac fluid extract with 300 ml of the water and acetic acid mixture and mix it thoroughly by shaking.
  • Then Filter the mixture, pass enough water in the filtration process in order to obtain 500 ml of filtrate.
  • Now add 100 ml of glycerin into the filtrate.
  • And dissolve 700 grams of sugar into the mixture.
  • Add enough water to obtain a product that reaches the measurement of 1000 ml.
  • Mix the solution thoroughly and then strain.
  • Store it in a bottle, jar, or any container and also label it.

ALERT:- This procedure steps are only for the source of information.

Do not get confused with the ipecac syrup and ipecac fluid extract. Do not contain these in the same jar or together or else it will cause harm. And even cause death because ipecac fluid extract is way more potent and aggressive than ipecac syrup.

Tips to use homemade emetic at home:-

You should be very careful while handling drugs and medications. They should be taken with proper prescription and medium. Any carelessness can harm the person who is taking or worse the condition. Some important tips while taking the homemade emetic or ipecac are:

  • By taking ipecac with milk or any other drink except water will reverse the action of ipecac or emetic that means it will not induce vomit or can give other irritable reactions and inhibits the real purpose.
  • When you are preparing it at home you should be very careful with the dosage of all ingredients. Do not overdose it.
  • After preparing it, store it in a jar and keep it at room temperature. Most importantly label it so anybody else doesn’t drink it.
  • Ipecac can be harmful when it is in direct contact with the skin. It causes irritation.
  • Ipecac is also harmful when it is inhaled so prepare it with full precautions and measures.
  • Before giving it to anybody at home kindly tell the person about the action and medical use.
  • Use as prescribed and kindly make sure what dosage you are giving.

Dosage:-

Dosage varies with the age of the person taking it. It should be taken by mouth. Overdose can be harmful. The dosage of homemade emetic or ipecac to avoid an accidental overdose is:

  • Adults should have 30 to 60 ml.
  • For children in between, 1 year and 2 year is 4 to 8 ml. If relief is not observed then repeat this after every 10 to 20 minutes.
  • For children after 2 years, 10 to 15 ml should be taken.

The time of action for ipecac is 10 minutes. The average time in which the stomach will be emptied is 20 minutes after action time.

Side effects of ipecac:-

Any emetic or Ipecac or any other homemade emetic which is manufactured and prepared for a specific purpose of treatment which is to induce vomiting and make the stomach clear either it is any poison or any other harmful thing. Emetics are marked safe when taken orally with water only. Changing in medium or route or overdosing can be lethal for the affected person.

  • Ipecac is unsafe when it comes in direct contact with the skin. It causes itching and irritation in the touched area.
  • Ipecac contains emetine which is an irritant for the respiratory tract. So if the ipecac is inhaled or smelled it can cause irritation in the respiratory tract or lungs.
  • Ipecac is unsafe for pregnant women and breastfeeding women. It increases the chance of miscarriage.
  • It is prohibited to use ipecac for the children under one year. Their stomach lining is more sensitive than adults; it will cause severe damage and even death in children if used in large amounts of high dose. First, consult a doc before using homemade emetic.
  • It is unsafe for people who have digestive tract problems like ulcers, infections, or any other disease like Crohn’s disease.
  • Ipecac should be prohibited to use in people who are unconscious or poisoned with chemicals like petrol, corrosives, or strychnine.
  • Ipecac is safe only for short term usage by mouth. It is not safe if taken for a long term in large amounts orally. It starts causing poisoning and damages the heart.
  • Misuse or overdose of ipecac can cause high damage, poisoning, and death.

Cholesterol is an organic molecule that is a type of lipid. It is a waxy liquid which is biosynthesized by animal cells. Cholesterol is found in structural components of cell membranes of animals. This fat-like substance is found in cells of the human body. The human body needs cholesterol for making substances that help in digesting food, making hormones, and vitamin D. Cholesterol is both good and bad in situations.

The liver produces lipoproteins that carry cholesterol and triglycerides into the blood vessel. There are two types of lipoprotein:

  1. LDL (LOW-DENSITY LIPOPROTEIN)
  2. HDL (HIGH-DENSITY LIPOPROTEIN)

When LDL cholesterol level gets high in the blood it is known as High Cholesterol. When high cholesterol levels are left untreated, it can develop many lethal diseases or can be a risk to various diseases like heart diseases, stroke, etc.

Bad cholesterol or LDL cholesterol carries cholesterol to the artery and make walls in the arteries. The wall build-up is known as ‘cholesterol plaque’. This limits the blood flow and can increase the risk of blood clots. One third of the adult population have elevated LDL Cholesterol levels.

HDL cholesterol:

HDL cholesterol or good cholesterol helps to return LDL to the liver so it can be excreted from the body. It also helps in the prevention of plaque in your arteries.
